Use this Survey Checklist to design a high-quality, accessible survey that will get you the data you need. 

Once you've reviewed the checklist, use this form to submit your draft survey to GSA's Customer Data and Survey Team (formerly, GSA Office of Customer Experience (OCE)) for review and approval.

The Customer Data and Survey Team must approve your survey before you can distribute it. You can expect a confirmation email, confirming receipt of your approval request, from the Customer Data and Survey Team within five (5) business days.
