Capabilities Statement:

Capabilities Statement should include your responses to all questions in Attachment 1: Additional Technical Questions. Capabilities should also include projects that are similar to the described requirements and that have occurred in the past 3 years.
Provide at least 2 relevant projects and include the following information:
1. Customer Name
2. Customer/Client POC Email
3. Total Contract Value
4. Period of Performance
5. Brief Description of Services Provided
6. Indicate if there is a CPARS (Contractor Performance Assessment Reporting System) Available
7. Contract number
8. Indication if you firm was a prime or subcontractor
9. If work was performed as a subcontractor, please indicate percentage of work/subcontract dollar value.
10. If the contract(s) were Indefinite-Delivery Indefinite-Quantity (IDIQ), please indicate the number of task orders awarded and the total dollar value of those task orders.
Please also provide your firm's average annual revenue for the past three years and the number of employees you currently have. In addition to the information requested above, interested parties may also provide comments on this potential acquisition regarding proposed contract type, incentive arrangements, evaluation criteria, or performance metrics.
